ABSTRACT
Treatment of patients with haemophilia have significantly evolved in the recent years. Among available therapies, “bypassing agents” (activated prothrombin complex concentrates or recombinant factor VIIa) are used in haemophilia patients with inhibitors. As their lab monitoring at post-infusion with factor VIII (FVIII) or IX (FIX) measurement is useless, other assays, such as thrombin generation or thromboelastometry, can be good candidate for their monitoring. During the webinar, the current state of the art for lab monitoring will be discussed.
Recently developed, the extended half-life FVIII/FIX products can present discrepancies between one-stage clotting or chromogenic FVIII/IX assays used for their monitoring. Furthermore, discrepant results have also been reported when different one-stage clotting assays employing different aPTT reagents. These issues are likely to complicate the laboratory monitoring of long-acting factor concentrates. During the webinar, paradigmatical examples will be presented along with the possible solutions to limit between-assay discrepancy.
New classes of drugs, not based on infusion of FVIII/IX molecules have already or are being introduced for prophylaxis of haemophilia patients. They include emicizumab, fitusiran or concizumab. The three drugs are able to increase thrombin generation in vivo and in vitro even in the absence of FVIII.

During the webinar, issues related to their laboratory monitoring will be discussed. With this webinar presented by Prof. Tripodi, clinicians, clinical pathologists, PhDs, clinical laboratory technicians and students will have the opportunity to increase their knowledge about the recent issues and challenges of laboratory monitoring of patients with haemophilia.

SPEAKER’S BIO

José Antonio Paramo
Prof. Armando Tripodi is Full Professor of Clinical Biochemistry at the Humanitas University Medical School in Milano, Italy and Established Investigator at the Angelo Bianchi Bonomi Hemophilia and Thrombosis Center, IRCCS Maggiore Hospital in Milano, where he manages the Laboratory for Development and Standardization of tests to diagnose disorders of hemostasis and monitoring of related treatment. He holds a PhD (1976) in Biological Sciences from the University of Milano.
In 1980 he joined the Department of Internal Medicine of the University of Milano as Assistant Professor of Clinical Biochemistry and was extensively involved in research programs aimed at the development, standardization and quality control of diagnostic procedures for diagnosis and management of patients with hemostatic diseases. In 2001 he was promoted as Associate Professor of Clinical Biochemistry and in 2006 became Full Professor of Clinical Biochemistry and was extensively involved in research programs aimed at investigating coagulation mechanisms of thrombogenesis. He has served as a member-elect of the Scientific and Standardization Committee of the International Society of Thrombosis and Haemostasis (2001-2006) and since then as co-chair of various Subcommittees. He served as partner and member of the Steering Committee of the European Action on Anticoagulation (1995-2006) funded by the European Union. He served as President (2001-2003) of the Italian Federation of Thrombosis Centers and is the current President of Italian Society for Thrombosis and Haemostasis. He is author of more than 400 scientific papers in books, international or national scientific journals and conference proceedings with an H index (Scopus) of 51.
